Short bowel syndrome (SBS) is a digestive disorder that deprives the sufferer of nutrients available in food consumed and essential for healthy living. It constitutes a significant threat to life and comes with very low survival rates among affected newborns. What causes this condition? What are the symptoms and what treatment options are available?

What’s to know about short bowel syndrome?

SBS is a disorder characterized by malabsorption of nutrients in foods. It is usually acquired as adults, but some children can be born with it. The poor absorption of nutrients that is experienced occurs as a result of the small intestine being too short. Food digestion and nutrient absorption mostly take place inside the small intestine. Short bowel syndrome typically crop ups when the small intestine becomes reduced to less than a third of its usual length of about 20 feet. It is seen in people who have had part of their small intestine removed or significantly damaged. The disorder may also develop when all or part of the large intestine has been removed.

Individuals suffering from SBS are not able to properly absorb protein, vitamins, minerals, calories, water and other nutrients present in foods that they eat. The nutrients that will not be properly absorbed will be determined based on what part of the small intestine that has been removed or become damaged. This disorder is a rather rare one, affecting only an estimated three of every million people. In the United States, adults numbering about 3,500 and children totaling around 9,000 are believed to be affected. Five-year survival rate among newborns having less than 10 percent of normal intestinal length is just about 20 percent.

What are the symptoms of SBS?

The major symptom that may first be noticed in a person suffering from short bowel syndrome is diarrhea. Such an individual will be passing loose, watery stools which will deprive him or her of essential nutrients. Diarrhea will result in dehydration, malnutrition and, eventually, to weight loss. These can end up in fatality if prompt medical help is not sought.

Causes of short bowel syndrome

SBS is a consequence of the small intestine being too short. The reduction in length, as stated previously, may be as a result of a removal of or damage to portions of the small intestine.   • Crohn's disease
  • Gastroschisis

  • Necrotizing enterocolitis
  • Internal hernia
  • Omphalocele
  • Injuries or trauma affecting the small intestines
  • Meconium ileus (ileum blockage by a newborn's thick and sticky stool

Surgery carried out to eliminate blood clots in the arteries supplying blood to the intestines may lead to short bowel syndrome. Bypass surgery performed to deal with obesity is another risk factor.

In some cases, children are born with unusually short small intestines or have portion of their bowel missing.

Short bowel syndrome treatments

SBS is treated using a variety of approaches, which include special diets, nutritional supplements and surgeries. A health professional will advice the best course of action, judging by the nutritional requirements of a patient. To determine the best treatment approach, a number of tests are usually done to assess the condition of a patient. They include blood tests, fecal fat tests, x-ray test, and a computerized tomography (CT) scan. Medical and family history will be assessed and physical examination is done.

Patients are usually advised to avoid foods with high fat content and those capable of causing diarrhea, including protein and sugar.
  • Supplements – Your doctor may recommend the use of certain vitamin and mineral supplements to help deal with short bowel syndrome. Among the nutritional supplements that could be useful are those containing vitamins A, D, E and K, folic acid, magnesium, calcium, and iron. You may be directed to use these alongside enteral or parenteral feeding.
  • Medications – It is common for drugs to be recommended for getting rid of symptoms associated to short bowel syndrome. These include H2 blockers and proton pump inhibitors which are used to regulate the amount of acid in the stomach. Codeine and loperamide are two of the medications that are used to control watery stools that people with SBS often experience. Other useful drugs include antibiotics, bile-salt binders, teduglutide, and anti-secretin agents.
  • Surgery and transplant – Depending on the extent of the problem, a surgical procedure may be advised to improve nutrient absorption in the small intestine. It may be needed to elongate the small intestine, narrow dilated segments of the small intestine, or eliminate blockage, among other reasons. An intestinal transplant could be recommended when the extent of the problem has become graver.

While there are several treatment options available for managing short bowel syndrome, these are not usually 100 percent effective. They only help to an extent and may come with certain side effects. For example, a small intestine transplant, which is thought to be highly effective, has been linked to post-surgical mortality rate as high as about 30 percent. This reality has raised the need for an alternative treatment method. Human growth hormone therapy has been suggested as being helpful to SBS patients as well

Some studies have shown the potential of growth hormone therapy in the management of short bowel syndrome. In one randomized, controlled study, the use of recombinant growth hormone (r-HGH), also known as somatropin, was observed to have significantly improved the absorption of protein, carbohydrates and energy. Human growth hormone therapy combined a specialized oral diet and an oral glutamine solution for a period of four weeks led to considerable reduction in the need for parenteral nutrition among patients in a controlled trial. The findings in this trial were said to have prompted approval of growth hormone for the management of SBS by the U.S. Food and Drug Administration.

However, it must be noted that findings from human and animal studies have been rather conflicting on the effectiveness of r-HGH in managing this disorder. These conflicts are said to have possibly been as a result of differences in methodology used for the different studies.

Short bowel syndrome, although rare, is a potentially life-threatening disorder that should be treated with all seriousness. Proper medical care is required to enhance likelihood of survival. A good number of the treatment options available do not offer total cure, but only help to manage the condition. Growth hormone therapy can improve nutrient absorption in the small intestine which is an aspect where SPS patients have challenges.
